Whether you are interested in Counseling or Coaching, the intake paperwork is the same.

Intake Forms

The intake forms should take about 15 minutes, and we recommend using a desktop or laptop computer for the best experience. Please ensure EVERY section is filled out and include as much information and detail as you are comfortable adding.

Once we receive and review your intake forms, a counselor will contact you to schedule an appointment within two BUSINESS days.
